1. Acceptance of Terms
By accessing or using DenyZero ("the platform") at denyzero.polsia.app, you agree to be bound by these Terms of Service. If you do not agree to these terms, do not use the platform.
2. Description of Service
DenyZero provides pre-submission claim analysis tools that help healthcare billing professionals identify potential denial risks before claims are submitted to payers. The platform includes:
- Claim checker for CSV-based claim analysis
- Denial feedback loop for rule gap detection
- Rule proposal system for continuous improvement
- Payer guideline monitoring
3. No Real PHI Restriction
You must not upload, transmit, or otherwise provide real Protected Health Information (PHI) to DenyZero.
The platform is currently designed for use with synthetic or de-identified data only. Real PHI includes any individually identifiable health information as defined by HIPAA, including but not limited to:
- Real patient names, addresses, or contact information
- Actual Social Security numbers or health plan beneficiary numbers
- Real medical record numbers or account numbers
- Actual dates of birth, admission, or discharge linked to individuals
If you accidentally upload real PHI, notify us immediately at denyzero@polsia.app. We will delete the data and confirm removal.
4. User Responsibilities
When using DenyZero, you agree to:
- Use only synthetic, fabricated, or properly de-identified data
- Maintain the confidentiality of your admin credentials
- Not attempt to circumvent security measures or access controls
- Not use the platform for any unlawful purpose
- Not interfere with or disrupt the platform's operation
- Comply with all applicable healthcare regulations in your jurisdiction
5. Disclaimer of Warranties
DenyZero is provided "as is" and "as available" without warranties of any kind, either express or implied.
DenyZero is not a substitute for professional medical coding, billing, or compliance advice. The platform's analysis is based on common denial patterns and rules and may not catch all potential issues. Users should:
- Not rely solely on DenyZero for claim submission decisions
- Maintain their own compliance and quality assurance processes
- Consult qualified healthcare billing professionals for complex cases
6. Limitation of Liability
To the maximum extent permitted by law, DenyZero and its operators shall not be liable for:
- Any claim denials, revenue losses, or financial damages arising from use of the platform
- Errors, inaccuracies, or omissions in the analysis results
- Loss of data or business interruption
- Any indirect, incidental, special, or consequential damages
7. Intellectual Property
The DenyZero platform, including its design, code, rule engine, and documentation, is the intellectual property of its operators. You may not copy, modify, distribute, or reverse-engineer any part of the platform without written permission.
8. Account Security
Access to admin features requires password authentication. You are responsible for:
- Keeping your credentials secure and not sharing them
- Notifying us immediately of any unauthorized access
- All activity that occurs under your credentials
Sessions automatically expire after 15 minutes of inactivity for security purposes.
9. Termination
We reserve the right to suspend or terminate access to DenyZero at any time, with or without cause. Upon termination, your right to use the platform ceases immediately. Provisions related to liability, intellectual property, and disclaimers survive termination.
10. Changes to Terms
We may modify these Terms of Service at any time. The "Last updated" date at the top indicates when changes were last made. Continued use of the platform after modifications constitutes acceptance of the updated terms.
11. Governing Law
These Terms of Service are governed by and construed in accordance with the laws of the United States. Any disputes arising from these terms shall be resolved in the appropriate courts.
